Introduction to Salesforce Health Cloud Assessment
Salesforce Health Cloud Assessments use the both Discovery Framework and OmniStudio to build more complex questionnaire paths. A healthcare practitioner can perform assessments in-person or over the phone. If your target is to create Question and Answer style assessment, then look no further than this.
Configuration
Prerequisite to configure Salesforce Health Cloud Assessment
Go through the below links to enable discovery framwork and associated permission and access.
Step 1 – Health Cloud Assessment Question Set
Go to App launcher and open Salesforce Health Cloud Assessment Question Set. The question set can have multiple questions. Provide a meaningful name which will represent the questions.

Step 2 – Health Cloud Assessment Question
Go to App launcher and open Salesforce Health Cloud Assessment Question. For this apart from Name and developer name, you need to fillup below fields
- Data Type
- Category
- Question Text
- Question Set
- Active (Should be checked)
- Response Value – Response Values is a required field when the assessment question is of the data type Radio, Radio Group, Select, or Multiselect. Enter response values with each value separated by a new line. Alternatively, for Select, Multiselect, and Radio, you can also define the response values later as Option Source in the OmniStudio designer.


Step 3
Click on the Select Question button which is upper right corner of the Assessment question’s list view and it will open a predefined OmniScript.

Step 4 – Create form
Go to the OmniScript and provide Name, Language, Type and subtype. Then select the Question set which you have created in the Step 1 from the lookup called Question Set(Upper left Corner of the Canvas). After that add Step and drag the question/questions in the step. You can add multiple questions in single step and/or multiple steps in an OmniScript. Then click the Build OmniScript button on the right upper corner. It will open the OmniScript in a separate tab however, It takes 30 -40 sec. So just wait for it. After opening, if you want to add some other components, then go ahead and add there.


Step 5 – Add Health Cloud Assessment Component
Now you need to add Salesforce Health Cloud Assessment component into patient lightning page. Go to the patient lightning page and add a custom tab in the main section. Following to that, search the Assessment component and add it to the newly created custom tab. Furthermore, Select the OmniScript Saved Session Fields and OmniProcess Fields to display on the Assessment card. Save it. Now your Health Cloud Assessment is ready to use.

Step 6 – Capture the response
Now go back to the Assessment tab and select all assessment from the list to show. Chose one of the Salesforce Assessment and click start to enter your assessment. After Assessment is submitted, you can see the completed Assessment under Completed section. If you want to see the response, then go to the completed assessment’s related section and you will find it.
