Introduction

I have started Salesforce Health Cloud implementation journey a couple of years back and I like to share my opinion and notes with you. I will try to touch every section of this vast industry and discuss implementation approaches, design considerations , x- factor etc. I hope this will help you to understand the topic more clearly.

What is Social Determinants?

Social Determinants of Health is one of the biggest challenges that today’s Healthcare industry is facing. During my Health cloud Certification, I understand the full meaning of this and more importantly how it has been impacting under privileged mass. 

The social determinants of health (SDH) are the non-medical factors that influence health outcomes. They are the conditions in which people are born, grow, work, live, and age, and the wider set of forces and systems shaping the conditions of daily life. These forces and systems include economic policies and systems, development agendas, social norms, social policies and political systems.

Simply put, the reason for which a person is not getting proper Healthcare is Social Determinants. This is a part of a larger Care Management area. Every country defined their own Social Determinants of Health (SDOH) For example, USA created five groups of SDOH,

• Economic Stability
• Education Access and Quality
• Health Care Access and Quality
• Neighborhood and Built Environment
• Social and Community Context

Here is couple of SDOH from European Union

• housing
• transport
• urban planning
• Employment
• occupational health
• adult education
• youth employment
• Financial security

Now the question is what are technologies available to solve this issue and answer to that question is that all big brands are offering solutions on this problem in their own ways. You can choose any one of these depending upon the criteria.

Social Determinants Data Model

Fictitious Social determinants issue which I will solve in the next blog

Lilian is group 2 diabetes patient who lives in a Northern part of Canada. She joined a special Government Diabetes care program which helps her to reduce the Glucose level. One of the task she has to follow as part of this program is to do exercise regularly. As a diligent person she has been doing that without any interruption. However, she started the program in summer and during fall she realizes that workout outside is going to be a very difficult specifically, in winter. She sends a survey which states her potential challenges.

Since she can have a tough time doing workout or running outside, join to gym or indoor facility due to low income is considered a determinant of health. Some of the questions in the survey refer to Lilian’s access to indoor Facility or gym (Care Determinant). Some of Lilian’s responses (Survey Response) indicate that she doesn’t have a budget/money (Care Barrier) and she can’t join the gym.

Government care program team logs a ticket (Case) to find a Gym membership (Task) covered by Government Health Plans. By getting a no-cost Gym membership (Care Intervention) for Lilian, they’ve helped her successfully overcome her money/low income barrier.
